After a concussion, many people are told their scan looks normal. While that is reassuring, it can also feel frustrating when symptoms continue. A concussion usually does not cause visible structural damage to the brain. Instead, it temporarily disrupts how the brain and body communicate with each other. The systems are intact, but they are not coordinating efficiently.
Because of this, symptoms can look very different from person to person. Some individuals develop headaches and neck discomfort, others feel dizzy in busy environments, and others mainly notice fatigue, sleep disturbance, anxiety, or mental slowing. All of these experiences are common after a concussion.
Why Symptoms Occur
Concussion affects more than just thinking or memory. Several body systems can be involved at the same time.
The neck
The forces that cause a concussion often irritate the joints and muscles of the cervical spine. The neck plays an important role in balance and spatial awareness, which is why headaches, pressure around the eyes, and dizziness are common.
Vision and balance
Your eyes and inner ear constantly send information to the brain to help you function in motion and in complex environments. After a concussion, the brain processes this information less efficiently. This is why places such as grocery stores, traffic, scrolling on a phone, or bright lighting can suddenly feel overwhelming.
Body regulation and energy
The nervous system that controls heart rate, energy level, and sleep can also become temporarily dysregulated. People often notice reduced stamina, lightheadedness, poor sleep, or feeling “tired but wired.” Digestive changes and appetite changes can also occur.
Anxiety sensations
After a concussion or prolonged illness, the brain’s regulation systems can become over-sensitive. Stress and threat circuits may remain in a high-alert state, activating the autonomic nervous system (fight-or-flight). This directly influences heart rate, breathing, muscle tension, and blood flow to the brain.
As a result, symptoms such as dizziness, head pressure, fatigue, brain fog, light sensitivity, and a racing heart can occur even without new injury. These are real physiological responses generated by the nervous system.
Anxiety is not just a reaction to symptoms, it can help drive them. Ongoing stress signaling can amplify pain pathways and disrupt regulation, keeping the nervous system in a protective state. As regulation improves, both physical symptoms and anxious sensations tend to settle together.
Nutrition and hydration
After a concussion the brain is working harder while using energy less efficiently. Skipping meals, dehydration, and large swings in blood sugar commonly worsen headaches, dizziness, and fatigue. Consistent meals, hydration, and regular sleep routines often help support recovery.
How Concussions Are Evaluated

Sometimes controlled exertional assessments are used to understand activity tolerance and help guide recovery planning. These evaluations help determine why symptoms are persisting and what areas need attention.

Recovery from concussion is usually gradual and guided. Most people improve when the affected systems are identified and supported appropriately. The goal is a safe and confident return to school, work, exercise, and daily activities.
Persistent symptoms do not mean permanent damage. In many cases they simply indicate that certain systems have not yet returned to normal regulation. With appropriate care and guidance, most individuals steadily return to their normal routines.
